Registered Nurse (RN) – Outpatient Primary Care

Job Type: Full-Time
Location: San Jose, CA
Salary Range: $52.72 – $64.30 per hour

 

Position Summary:

We are seeking a compassionate and experienced Registered Nurse (RN) to join our outpatient primary care team. The RN will play a key role in delivering integrated nursing services, supporting providers, and leading clinical initiatives to ensure high-quality patient care.

 

Key Responsibilities:

• Triage patient calls and walk-ins, assess needs, and provide appropriate care or referrals.
• Review and follow up on lab results, imaging, and other diagnostics in collaboration with providers.
• Administer vaccines (pediatric and adult), perform wound care, venipuncture, and point-of-care testing.
• Provide health education and support to patients and families.
• Supervise and mentor nursing students and ancillary staff.
• Participate in off-site health events and ensure proper vaccine handling and documentation.
• Assist with onboarding health screenings for new employees.
• Support the development of training materials and patient education resources.
• Maintain accurate and timely documentation in the Electronic Health Record (EHR) system.

Qualifications:

• Education: Bachelor’s degree in nursing (BSN) preferred.
• Experience: Minimum of 2 years of nursing experience, preferably in outpatient or community health settings.
• Licensure: Active RN license in the State of California.
• Certifications: CPR required; ACLS and PALS preferred.
• Technical Skills: Proficiency in EHR systems and Microsoft Office.
• Soft Skills: Strong communication, cultural sensitivity, and ability to work in a multidisciplinary team.
