Care Coordinator/Health Coach:
Join our team as a Clinical Care and Health Coaching Specialist! A Clinical Care and Health Coaching Specialist at Bastion Health is a Care Coordinator who specializes in Health Coaching. Essential Responsibilities:
Assess patients' needs and develop individualized care plans
Coordinate and schedule medical appointments, procedures, and tests
Collaborate with physicians, nurse practitioners, and healthcare professionals to streamline care
Educate patients and families about their conditions, treatment options, and medications alining with the providers' assessment and plan
Closely monitor patients' progress
Maintain accurate and up-to-date patient records
Participate in quality improvement activities
